- Payment for Non-Included Treatments: Any treatment not covered by the membership plan will require payment at the time of service.
- Combined with Other Offers: The benefits of the FFD Membership plan cannot be combined with insurance, other offers or discounts.
- Special Procedures: The membership plan does not cover procedures performed by specialists outside of our practice.
- Preventive Care Coverage: The plan covers preventive services, with no waiting periods, deductibles, copays, or annual maximums.
- Membership Duration and Price Transparency: Membership is valid for 12 months from the date of purchase and includes 100% price transparency for covered services.
