How long does the membership last?
- Not Insurance: The FFD membership plan is not insurance and is not a qualified health plan under the Affordable Care Act.
- 30-Day Money-Back Guarantee: Your are eligible for a full refund within 30 days of purchase, provided no benefits of the plan have been used. Refund requests after 30 days will be considered at the discretion of our practice.
- Payment for Non-Included Treatments: Any treatment not covered by the membership plan will require payment at the time of service.
- Combination with Other Offers: The benefits of the FFD membership plan cannot be combined with insurance or other offers or discounts.
- Specialist Procedures: The membership plan does not cover procedures performed by specialists outside of our practice.
- Preventive Care Coverage: The plan covers preventive services, with no waiting periods, deductibles, copays, or annual maximums.
